Penile enhancement: Does size matter?
Is penis size important for a partner’s sexual satisfaction? Yes, it is.
Below you will find six studies and surveys about the ideal penis size in terms of sexual satisfaction, which prove that a penis enlargement can make sense and even be necessary in some cases. Around 10,000 women were surveyed in total and their responses revealed that the optimum penis size when erect is between approx. 17 to 19 cm.
2025: Deutsche Gesellschaft für Mann und Gesundheit (German Society for Man and Health), survey of around 1,500 women:

2023: Lifestyle: Editors from Germany’s biggest question-and-answer platform Gutefrage GmbH asked women whether they talk about the penis size of their current and former partners with their female friends:
- 63% of the women surveyed said that they discuss their current partner’s penis size with their female friends.
When asked what the minimum penis size should be:
- 45% of the women said 15-17 cm
- 44% said 18-21 cm
2020: Lifestyle: In the GQ Magazine issue from 2 July 2020, a survey of 4,871 women was published in which they were asked what penis size would have been necessary for them to reach orgasm in the past.
- The average value of their responses was 20.32 cm.
2019: According to a study by the University of Ottawa, which was published in the scientific journal ‘Proceedings of the National Academy of Sciences’, women instinctively prefer a partner with a larger penis given the choice.
2013: On 9 April 2013, renowned medical news website ‘Medical News Today’ published an article entitled ‘Size does matter’, quoting a study by the Australian National University, which found out that:
- 16.2 cm is the ideal average penis size.
2011: Lifestyle: In their 9/2011 issue, Süddeutsche Zeitung magazine published a recorded, open and honest conversation between six women on the significance of penis size with the headline Die ungeschminkte Wahrheit (The Naked Truth). The overall tenor of the conversation was that:
- It’s a shame that men’s magazines are writing articles about how penis size doesn’t matter. The only people who could write anything like that is men – in order to reassure themselves.
- The women surveyed said they wished it would be mandatory for men to state their penis size on online dating websites. As penis size is of vital importance for sexual satisfaction, there needs to be a way for women to find out in advance whether her dating partner has a size that will help her enjoy a fulfilling sex life.
Conclusion:
It was often claimed that the average penis size was just 12.4 cm when erect. But how does that align with the study results? Have women become unashamedly demanding all of a sudden?! No, not at all! 12.4 cm has always been ridiculous, especially considering that the condom norm in Europe specifies a minimum length of 15.6 cm.
It was probably the “old white men” of the patriarchy who scaled down this average value over the past few decades to repress desire in women and keep female sexuality in check. This theory is supported by another study: on 14 February 2023, Stanford University published a meta-study about average penis size.
They found out that in the last 30 years, the average penis size has reportedly increased by 24%, even in identical men. As this cannot be the case and there are no solid reasons to back it up, it is more likely that the “studies” of the past were actively downscaled and penis sizes weren’t actually that large to begin with.

Further information about penis size
The question of whether penis size is important is in itself an insult to women, and actually extremely discriminatory.
Sexual intercourse between a man and a woman is a mechanical process that involves a “rod” being pushed into a “passage” and moved up and down inside it for a while. Anyone who would dare to claim that the “passage” alone is to blame for any problems with intercourse, and the shape and size of the “rod” isn’t important, would be discriminating against women and completely out of touch with reality.
Even Charles Darwin, the great evolutionary biologist, established that it only comes down to size and size alone. After all, the shape and structure of the penis and the way that intercourse takes place between man and woman was developed for a very specific purpose: the penis isn’t just a rod; it is made up of a shaft and a much wider head, which also has a rim that projects even further outwards. Basically, it is built like a shovel.
But the reproduction process doesn’t involve the man inserting his penis into the woman and simply releasing semen before pulling it out again. Intercourse consists of an up-and-down movement, an in-and-out, lasting several minutes. This is also carried out with a significant driving force, which most women also happen to find pleasurable.
Why is that? And why does it bring so much pleasure although it involves vigorous impact?
The answer is that there’s a purpose behind the act of intercourse, and the key to this can be found in our biological origins: after all, the organ and the process are designed to scoop out any semen that may have previously been deposited by another male and to place one’s own semen as far into the female’s reproductive tract as possible so as to protect it from further “shovelling” attempts by anyone else. “Survival of the fittest” is a phrase that originated from Darwinian evolutionary theory.
Of course these days women don’t usually have intercourse with multiple partners in succession. But for our ancestors (we are partly descended from bonobo apes), that was still the case. And sexual instincts are deeply embedded in our brain.
Back then, it was important for the female ape that intercourse was performed with great vigour. And the female also needed to know who penetrated the deepest: it was important for her to get on well with this male because she needed him for protection, food and to help bring up their offspring.
This explains why women take pleasure in vigorous intercourse with large (not oversized!) penises – because their instinct still demands it.
The pleasurable feelings and sensations that make us aroused during intercourse and its climax are actually nature’s way of thanking us for a successful reproductive process.
And the following contradiction shows how deeply these instincts are ingrained in us and how much women pay attention to these processes without realising: while it’s usually not a problem for the woman not to reach climax, no woman would be impressed if the man climaxed too early.
Why? Because as far as reproduction is concerned, this whole process would be a waste of energy as the “shovelling” process wouldn’t last long enough and the sperm could not be inserted deep enough.
The decades-long claim made by “old white men” that only the entrance to the vagina was supplied with nerves – which denied women of their needs or any pleasure of their own – is of course completely absurd. The idea behind it was to repress women and their desires.
“5 cm are enough for reproduction” is what the urologists used to say. Seriously though?! The average German couple has 1.5 children and will have sexual intercourse 12,000 times in their lives so it’s clear that there’s a lot more to sex than reproduction…
But thankfully the old attitudes are changing. The days of “old, white men” laying down the rules are history.
What still shocks us is how much some die-hard feminists are contributing to further suppressing the needs of women by claiming that size doesn’t matter – because they believe that men would have too much power over women if their penis was actually important.
And a lot of women, due to a lack of experience and knowledge, would simply repeat what men have been instilling in them for decades. After all, it is a scientific fact that if you were to ask the average woman whether size matters and she were to answer “no”, she would really mean: “I don’t know”.
This is because the penis follows a normal Gaussian distribution of physical features – only one in ten is considerably large or considerably small. But as the average German women has had only five sexual partners in her life, the majority has likely never experienced a very small or a very large penis. Plus, if her partner were sitting next to her when this question was asked, it’s highly unlikely that she would be entirely honest, and it would take a lot of guts to tell the actual truth.
Penis enlargement part 1: Lengthening
Penis enlargement surgery consists of penis lengthening and girth enhancement – in one procedure. This is because combining the two achieves a much better result.
We have been performing penis enlargements for around 15 years – under sedation and as outpatient surgery. And it is our experience and expertise that make this possible.
It goes without saying that the anaesthetist is always on hand and is solely responsible for putting the patient under sedation, not the surgeon. So the reason why we use sedation rather than a general anaesthetic is not to eliminate the need for an anaesthetist during the penis enlargement procedure, but because it’s a lot more pleasant for the patient and a gentler experience overall.
The first part of the procedure is the lengthening of the penis. We do this through a very small incision, which is about the width of an index finger. This means that our penis enlargement procedure involves minimally invasive keyhole surgery.
We release the penis ligament at the front, at the so-called symphysis – the connective cartilage between both pelvic bones (os coxae). During the penis enlargement operation, the rear ligament remains untouched.
Why? Because it would be extremely dangerous to detach the rear part and that would also cause instability, which is exactly what we don’t want in a penis enlargement.
It’s important to point out the difference to a very old surgical penis enlargement technique that achieved poor results: by “front” ligament, we mean the front internal ligament and not just the superficial front ligament.
At the front we create a tissue block by laterally inserting the patient’s own tissue using suture techniques and materials (autologous tissue only) specially developed by us. As we are highly experienced and well trained in this technique, we don’t need to use any foreign materials to achieve success. By forgoing the use of foreign materials, we reduce the risk of the operation while achieving the same excellent result.
This block has two tasks: it forms the new anchorage of the penis to the pubic symphysis. And that is its key function: keeping the body of the penis in its new, lengthened position.
Why do we do this? With our penis enlargement procedure, we use a geometric trick: we straighten the oblique and curved course of the penis inside the body to create an overhang. This overhang forms the extension on the outer part – the section of the overall organ that men would refer to as their penis.
In fact, we bring part of the inner section of the penis out – but without altering or damaging the root of the penis in any way.
Let’s take a look at why we do it this way:
- The base of the penis, i.e., the area where the urethra and the vessels and nerves enter the penis, is not touched during our penis enlargement procedure.
- We extensively restructure its front inner suspension in order to optimise it and increase its size.
So we are not making an incision into the actual penis organ itself – which eliminates any risks that this would entail.
The size of the increase in penis length depends on the inclination and length of the symphysis, as well as on the existing ligament structures – which is different for every individual.
We used to say that the ratio between the external and internal sections was 50:50, so if you happen to come across that on other practitioners’ websites, then you’ll know who has been copying our words to trick you into believing that they use the same technique as we do.
Penis enlargement part 2: Girth augmentation
We perform girth augmentation using autologous fat and stem cells. Only surgeons who have mastered this technique will achieve the best results. The reasons why we use the patient’s own body fat are explained further below. Girth augmentation is the more difficult part of the penis enlargement procedure because a lot of things need to be taken into consideration and the surgeon requires considerable experience to ensure a good, permanent result.
If, during our enlargement procedure, we were to remove fat from your body and inject it under the skin of the penis, this would last six months at the most before breaking down completely. This means you should be extremely cautious if you ever come across a clinic that wants to charge you for repeat injections.
Unlike them, we remove living fatty tissue from the inside of the thigh. In an extensive process, this is specially prepared, cleaned and then injected under the skin of the penis to ensure a consistent, even result once the healing process is complete.
Some patients are worried that the small amount of fat that they have there might not be enough. But let us reassure you: in the case of a normal liposuction treatment (which you’ve no doubt seen on TV before), three to six litres of fat would be removed from your body. For a girth enhancement, we only need around 200 millilitres.
All tissue, including fatty tissue, contains stem cells. The fact that stem cells are transplanted here is not a special feature of the procedure – as some doctors like to claim in their marketing communications – but an automatic consequence.
We have only ever used autologous tissue in our girth enhancement procedures – and with great success.
When it comes to penis enlargement, achieving a permanent result using the patient’s own tissue requires a lot of experience and expertise.
From a technical point of view, it would be easier to insert foreign material underneath the skin of the penis, such as industrially manufactured skin strips or small pieces of silicone. But foreign material behaves differently to a patient’s own tissue and can often feel different, while posing additional risks. In the case of many materials, there is a high risk of infection, as well as a risk that the result after the enlargement will not be aesthetically pleasing.
We refrain from using any foreign materials and don’t claim that inserting pieces of silicone or skin strips is a new technique merely to entice patients.

Another important point we’d like to make is that there are many doctors out there who claim that they’ve been trained by us, have worked with us as an assistant, or learnt how to perform this operation from us in a workshop. But they are all lies as we don’t train other surgeons.

How safe is the operation?
Safety is the prerequisite for a good result, which is why we refuse to use any solid or liquid fillers or implants.
Even though there are surgeons out there trying to sell them to patients on the internet as “state-of-the-art”, “innovative” or even “cheaper” than the rest, we have known for years, and it has been proven by studies, that:
- liquid fillers (silicone, paraffin, etc.) can cause significant disfigurement;
- synthetic skin strips (such as AlloDerm) have even been banned for use in penis enlargements by the manufacturers themselves;
- solid implants (silicone, Elist, Matrix, Penuma, etc.) can cause severe injuries, capsular fibrosis with massive shrinkage of the penis and severe infections.
Are there any lower cost options?
Here’s a number that will shock you: on the TV channel Sky Documentaries, journalist Cherry Healey reported on 25 June 2022 that UK patients alone have to spend around 40 million euros more per year to have botched plastic surgery that was carried out abroad (usually in Turkey) corrected in their home country.
You cannot put a price on medical expertise, safety, the highest quality materials, capable staff etc. and booking such procedures is a lot different to buying a washing machine or a TV. Expecting a discount or lower prices with the same level of quality and safety is naive and actually rather unreasonable when it comes to something like penis enlargement surgery.
In fact, we actually offer the surgery at a competitive rate as we are able to take advantage of a number of discounts when purchasing certain consumables due to the high number of operations we carry out and can pass these savings on to our patients.
Risks of Penis Enlargement
When performed by a surgeon experienced in this procedure, penis enlargement is very low-risk.
The biggest risk—something we believe you have understood from our explanations of the technique and why you should choose us—is ending up with the wrong doctor. Unfortunately, this risk is still very high when it comes to penis enlargement.

Sometimes, urologists instill incredible fear in men who turn to them for advice before undergoing a surgical penis enlargement.

A urologist treats the urinary tract in both men and women. This is often misunderstood because so many older men visit urologists. However, they are only there because an enlarged prostate makes urination difficult.
A urologist should honestly answer your question with: “I don’t know.” Unfortunately, this is still a sentence that many doctors find very difficult to say.
